I've seen pictures of Tsuna from Katekyo Hitman Reborn in a cape with the roman numeral I on it. Is that just a fan thing or does he get it at some point in the anime or manga?

He will. It's part of his "box weapon". Best episode tbh, Tsuna looked badass.
